Application Process
STEP 1
Complete Information Request
An Information Request and Tour Form can be completed HERE or contact a representative at connect@29acres.org or at 214-550-8831.
STEP 2
Attend Informational Tour
An Informational Tour can be conducted over the phone or in person. The purpose of the tour is for applicants, families and caregivers to have a full understanding of 29 Acres. This includes a review of our supports services, contracts/billing, scheduling and answering any other questions.
STEP 3
Application, Documents and Fee
The applicant and their support team will complete the application online (sent via email), submit any additional required documents and submit the non-refundable Application Fee of $300.
STEP 4
Family Interview
The 29 Acres applicant and their support team will come for an interview. This process will include a review of the application, a determination of level of support needed and begin the process of designing an individualized life plan or ILP. This is a 3 hour process minimum.
STEP 5
Program Review, Billing, Contract
The 29 Acres applicant and their support team will return to review services and the applicant’s Individualized Plan. The Service Contract will be completed at this time. This process will take approximately 2 hours.
STEP 6
30 Day Review
The first 30 days of client support services will be used to modify goals, objectives & teaching strategies. After this first month Enrich 29 will request a brief meeting to review the ILP for additional changes on goals and teaching strategies.
STEP 7
6 Month Meeting
Although outcomes will be monitored year-round & communicated to family and caretakers, every 6 months (at a minimum) parent/guardian/client meetings will be requested for the purpose of monitoring progress and updating the clients Individualized Life Plan.
